II. DESCRIPTION OF SERVICES
Restore Hope Physical Therapy, PLLC provides specialized pelvic floor physical therapy services to adults of all genders. Our services include, but are not limited to:
- Pelvic floor physical therapy evaluations and treatments
- Manual therapies including myofascial, cranio-sacral, and visceral mobilization
- Treatment for pelvic pain, painful sex, low back pain, urinary incontinence, pregnancy or postpartum related issues, and other conditions related to the pelvic floor
- Exercise prescription and patient education
- Biofeedback for bowel/bladder dysfunction and prolapse
The specific services provided to each patient will be determined based on their individual needs and treatment plan as developed by Dr. Hope Cunningham and the patient.

IV. APPOINTMENT POLICY
A. Scheduling
Appointments can be scheduled by phone at 202-990-7177, by email at DrHope@restorehopept.com, or through our online scheduling system when available.
B. Cancellations and Rescheduling
We understand that circumstances may arise that require you to cancel or reschedule your appointment. We request that you provide at least 48 hours’ notice for any cancellation or rescheduling. This allows us to offer the appointment time to another patient who may be waiting for care.
C. Late Cancellation/No-Show Fee
Appointments cancelled with less than 48 hours’ notice or missed appointments (“no-shows”) may be subject to a fee of $100. This fee is not covered by insurance and will be the patient’s responsibility.
D. Late Arrivals
If you arrive late for your scheduled appointment, your session may be shortened to accommodate our schedule, but you will be responsible for payment of the full session.
V. PAYMENT AND INSURANCE
A. Payment
Payment is expected at the time services are rendered unless prior arrangements have been made. We accept cash, checks, and major credit cards.
B. Outstanding Balances
Patients with outstanding balances may be required to make payment arrangements before scheduling additional appointments. Accounts with balances over 90 days past due may be referred to a collection agency.
VI. PATIENT RESPONSIBILITIES
As a patient of Restore Hope Physical Therapy, you agree to:
- Provide accurate and complete information about your health history, medications, and any changes in your condition
- Follow the treatment plan recommended by your physical therapist
- Attend scheduled appointments and actively participate in your care
- Notify us of any concerns or problems with your treatment
- Update us about any changes in your contact or insurance information
- Pay for services in a timely manner as outlined in our payment policy
